Can anyone tell me where I can print off a copy of Form UNN 1017? I looked on the Immigration web site under forms but could not find it. Appreciate any help I can get.
My son-in-law had his medical done in Canada and sent in paperwork for outside application. On the phone today he was told we were missing the IMM 1017. I can find the forms needed for an inside applicaton but not for the outside.
Elly, the IMM 1017 has to be completed and signed by the doctor, so your son should either telephone or visit the doctor's office in person and ask them to complete the form and send it in.
