The Authority to Release Personal Information to a Designated Individual Form (IMM 5475)
Purpose: For appointing a designated individual to enquire about the status of an application and / or changing the address information of the applicant
Document Requirements:
- The system does not request for this document even if the applicant has indicated that they are designating an individual
- The applicant must provide the document in the open field if they have indicated that they would like to designate an individual
- The applicant would need to sign the document along with their spouse or common-law partner (if applicable) if they wish to release personal information to the same designated individual
- Dependent children who are 18 years of age or older would need to complete their own copies if they wish to authorise the release of their information to a designated individual
The Individuals who would need to submit this documentation:
- The principal applicant
- Their spouse or common-law partner (if applicable)
- The dependent children, including non-accompanying dependent children
